St. Clement’s Surgery: NHS and Private Care

At St. Clement’s Surgery, we are committed to providing comprehensive healthcare to all our registered NHS patients. We understand that some patients may also choose to access private healthcare, and we aim to navigate this in line with national guidance to ensure clear separation between NHS and private care.

Our approach is guided by the NHS “principle of separation” and the General Medical Services (GMS) Regulations, which both clearly state that NHS resources should not be used to subsidise private care.

Key points regarding the division of NHS and Private Work at St. Clement’s Surgery:

  • Shared Care with Private Providers: The BMA guidance advises against “shared care” with private providers where NHS GPs are expected to deliver parts of a private episode of care, particularly for treatments not available on the NHS or where a patient has bypassed NHS waiting lists by paying privately. Such patients are free to arrange shared care with a private GP and their private specialist, and their position on NHS waiting lists will remain unaffected.
  • Clinical Referrals: A standard clinical referral letter that a GP would make to either an NHS or private provider is considered part of NHS “treatment” and is therefore not chargeable.
  • Administrative Documents and Reports for Private Providers: The GMS Regulations allow NHS GPs to charge for the completion of medical reports and administrative documents (e.g., specific forms required by a private provider that go beyond a standard clinical referral letter). If a private provider expects the surgery to fill out additional forms or provide extensive administrative support beyond routine clinical referrals, this may be viewed as non-NHS work, and a fee may be charged to the private company for this service.
  • Patient’s Right to NHS Care: A patient’s decision to access private healthcare for a specific condition does not affect their entitlement to receive other NHS care for which they meet the commissioning criteria.

We encourage patients considering private healthcare to discuss their plans with their private provider regarding how their ongoing care, including any necessary tests, prescriptions, or follow-up, will be managed and funded.
