St Clements Surgery is the college doctor for students studying at Mansfield College. 

Please contact the surgery once you have registered, to book an appointment for a new patient health check. 

Ensure you have provided the surgery with an up-to-date contact number, to enable us to contact you if we need to. 

The surgery is able to provide advice and guidance in areas, such as: 

  • contraception
  • sexual health advice/concerns
  • travel clinics
  • mental health issues
  • examination medical certificates
  • general health issues
 

Online Registration

To begin the online registration process, please complete all of the forms listed below. Once you have filled out the form, press 'send' and the form will be emailed to the surgery. 

When registering, you will be asked to complete some forms, including a new patient health questionnaire. This is because it can take some time for your medical records to be transferred over.

Please allow 48 hours for your registration to be processed, once all forms are completed and you have provided proof of ID/address.
